In a long career in software development, Prakash has helped build patient management and hospital management systems on multiple continents. Prior to joining BaseHealth, he served as the Vice President of Development at Hara Software, a KPCB-backed startup that has defined and led the energy management market.
He also spent more than a decade at Oracle, where he led teams that built cutting-edge performance management solutions for distributed Enterprise applications.
Prakash has always been intrigued by the promise of data-driven medical care, particularly its role in prevention. He joined BaseHealth in March 2013.
Prakash holds a bachelor’s degree in Engineering and a master’s degree in business.
Hossein Fakhrai-Rad, Ph.D.
Founder & Chief Scientific Officer
After years of biotechnology and molecular biology research at the Karolinska Institutet in Sweden and the Stanford Genome Technology Center, Hossein founded BaseHealth to help decrease individuals’ risk of preventable disease. Hossein oversees BaseHealth’s long-term vision and growth strategy.
In addition to his passion for genomics, personalized medicine, and wellness, Hossein is a proven life sciences entrepreneur. His first startup, ParAllele Bioscience, was acquired in 2005 by Affymetrix, a global leader in genomic analysis tools.
Hossein earned his bachelor’s degree in molecular biology and biotechnology from Stockholm University in Sweden, and his Ph.D. in medical genetics from the Karolinska Institute in Sweden. In 2001, he held a postdoctoral research fellowship at the Stanford Genome Technology Center.
Mehrdad Rezaee, MD, Ph.D
Chief Medical Officer
Dr. Rezaee is an interventional cardiologist with over 20 years of research experience, ranging from basic cell and molecular biology to translational studies.
He has been integral to the growth and direction of BaseHealth from the start, especially as it pertains to building relationships with doctors and providing guidance to our scientific and medical teams.
Prior to assuming a predominantly clinical role in building world-class cardiac programs in community hospitals and clinics across Silicon Valley, Dr. Rezaee served several fellowships at Stanford Medical Center where he also served as Director of pre-clinical cardiovascular research.
Dr. Rezaee completed a medical scientist training program and was recipient of an outstanding dissertation and NIH training research awards. He received his Ph.D. from the Medical College of Wisconsin and completed his internship and residency at Harvard Medical School.
Director of Human Resources
Dianna has over 25 years human resources management and operations experience, and has worked with across industries around the world in the areas of compensation, leadership development, recruiting, talent management, and human resources information systems. Prior to joining BaseHealth, she held senior global leadership roles at National Semiconductor, was Vice President of Operations and HR at Hara Software, Director of HR at Beceem Communications, and Managing Director of HR Operations at Lam Research. She is a founding partner of The Pendolino Group.
Dianna has a Bachelor degree in Psychology from the University of Maryland and an MBA in Technology Management. Dianna is a board advisor to several for-profit and not-for-profit organizations.
Chief Financial Officer
Kazu has more than twenty years of experience in Financial Management spanning startups to large international corporations in the US, Japan and Brazil. As a senior finance executive, he has helped raise over US$150M for start-ups such as Genesys Labs, Centerbeam Inc. and Reply.com in the Bay Area.
Prior to joining BaseHealth, Kazu served as Director of Business Planning and Strategies at Visa International and Director of Business Planning and Strategies at Prestige International, Japan’s leading value added CRM solutions provider during Pre-IPOs. Most recently, Kazu worked in Sao Paulo, Brazil to broker M&A deals among Brazilian and foreign companies in Logistic Services. Kazu earned B.A in Economics from Doshisha University in Kyoto, Japan and an MBA from East Carolina University.
